Facial Fillers Overview - Radiesse®, Restylane ®and Perlane®, Juvederm®, and Juvederm®VOLUMA
Concepts in facial aging have historically focused on loss of skin elasticity and strength of deeper tissue leading to a sagging appearance along the jaw-line and prominence of the bones around the eyes. More recently, plastic surgeons have implicated loss of volume and bone regression as contributors to facial aging. Facial fillers (aka dermal fillers) are a recent addition to plastic surgeon’s tool kit to address volume loss, therefore restoring fullness and rejuvenating a patient’s appearance. Facial fillers are now the second most common non-surgical cosmetic procedure in the US.
At our center, fillers are used to build up cheeks, soften lines around the eyes and mouth and accentuate lips. Generally, any fold or depression in the face can be corrected with the proper application of facial fillers. I routinely use Radiesse, Restylane, Perlane, Juvederm Ultra and Juvederm Voluma. The many products available each have advantages and disadvantages which can be overwhelming for patients. It is important to select an experienced plastic surgeon and at the Gardens Cosmetic Center, we understand these products and can choose which will work best for you and the areas you would like to improve.
Consultations involve identifying your specific concerns, explaining what to expect and discussing risks before any filler is used. Once a plan is established, the skin can be numbed with a topical cream and cooled with icepacks. Injections typically have minimal discomfort and progress is regularly checked with the patient using a mirror. The effects are immediate and there is very little downtime. There are few, if any, side effects and the most common reaction is subtle swelling for a few days. We invite patients to return for a follow up in 2 weeks. By that time, the area injected has “settled down” and the long term outcome can be evaluated. Patients getting filler at our center generally look more relaxed, rested, rejuvenated and have a more youthful appearance overall. Radiesse® Dermal Filler
Deep volume for cheek augmentation
Using fillers to alter facial bony structure has been a powerful alternative to placing filler under a wrinkle. A powerful appearance of youth is achieved when the face has a heart shape. The cheeks are the rounded wide part and the face tapers down towards the chin, which is at the point. In order to accentuate and recreate this heart shaped look, filler is injected deep along to bone of the cheeks and sometimes in the chin. This will augment the bone and form a higher foundation to plump the soft tissues of the face.
Radiesse is a product we often use at the center for cheek augmentation. It is made from calcium hydroxyapetite which is the same composition as the mineral portion of bone. It is mixed with lidocaine and injections are largely pain free with minimal down time. The results can last up to 12 months and some studies are showing it may last even longer. Our extensive experience in using Radiesse for cheek augmentation has shown it to be a safe and reliable product.
Radiesse can also be used routinely in other areas as well. It is a heartier material and can give strong support to push out deep and sever wrinkles. The lines from our nose to the sides of the mouth and lines under the corners of our mouth are greatly improved with Radiesse.
Restylane ®and Perlane® Injectable Fillers
Restylane and Perlane are hyaluronic acid facial fillers from the same family of products. Perlane is slightly denser and can give the added boost to sever facial wrinkles to get the results you want. The areas most commonly injected with these products are under the eye to erase a hollow look, in the nasolabial folds, the marionette lines and in the lips. Restylane is ideal for the area under the eye since it does not expand and reduces the risk of an unwanted swollen, puffy appearance. Lidocaine is pre-mixed with the hyaluronic acid gel to improve your comfort with injection. The procedure is generally done in less than 20 minutes with minimal side effects. Patients can get back to normal activities with instant results.
Juvederm Ultra and Ultra Plus are hyaluronic gel fillers used in many areas of the face. The nasolabial folds, marionette lines and the lips are the most common areas injected with Juvederm Ultra. The “plus” in ultra plus indicates a denser gel which is used when lines are particularly deep and more lifting power is needed to get the best result. Juvederm flows smoothly making the material easy to inject into just the right spot needed. It tends to absorb some water creating a soft look when healing is complete. Lidocaine is already mixed into the gel to ensure minimal discomfort with the injections. Overall, getting a facial filler with Juvederm Ultra is easy with minimal risk and side effects.
The latest advance in facial fillers Are there new products available?
Loss of facial volume is a major contributor to the lines and contours that are associated with looking older. Facial fillers (aka dermal fillers) have been a great way to replace lost volume and with physicians using them for many years now, we know this non-invasive technique is very safe. At the center, we routinely use Radiesse, Restylane, Perlane and Juvederm Ultra. These are used in various places on the face with advantages and disadvantages for each product. Common applications are to place fillers below lines around the nose and mouth.
More advanced techniques seek to change facial geometry. An inverted triangle with the cheeks as the widest part tapering to the chin is thought to be a powerful look of youth, femininity and beauty. Fillers are placed in deeper levels below the skin to build up the cheeks under the eyes across to the cheekbones and a new filler has been introduced specifically for this purpose.
The name of this new product is Juvederm Voluma. It was recently approved by the FDA and is the first filler specifically approved for replacing volume in the cheeks. It does fill a void in materials currently available for cheek augmentation since it is reversible, has numbing medicine mixed into it and may last up-to 2 years with no down time. Extensive trials have demonstrated the product's safety and I will currently offer Juvederm Voluma to our patients.
